This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment to your privacy

The Counseling Kit, LLC is required by the Health Insurance Portability and Accountability Act (HIPAA) and Florida law to maintain the privacy of your protected health information ("PHI"), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your information

Treatment. We may use your PHI to provide, coordinate, or manage your mental health care.

Payment. We may use and disclose PHI to obtain payment for services provided to you.

Health care operations. We may use PHI for activities necessary to operate the practice, such as quality assessment and clinical supervision required by Florida law for Registered Mental Health Counselor Interns.

Disclosures required or permitted by law

We may disclose PHI without your authorization in limited situations, including: when required by law; to prevent a serious threat to health or safety; to report suspected abuse, neglect, or domestic violence; in response to a court order or subpoena; and for public health activities.

Your rights

  • Request access to and a copy of your records.
  • Request an amendment to your records.
  • Request an accounting of certain disclosures we have made.
  • Request restrictions on certain uses and disclosures of your PHI.
  • Request to receive confidential communications by alternative means or at alternative locations.
  • Receive a paper copy of this notice upon request.
  • File a complaint if you believe your privacy rights have been violated.

Changes to this notice

We reserve the right to change the terms of this notice at any time. The new notice will apply to all PHI we maintain. The current notice will always be available upon request and posted in our practice.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with The Counseling Kit, LLC or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
